Idaho Home Choice Money Follows the Person Demonstration Grant will help people transition from an institution (skilled nursing facility, intermediate care facilities or institutions for mental diseases) to community living in an apartment, private home or community setting such as a certified family home or residential assisted living facility. MFP initiatives increase outreach to individuals in institutions and decrease barriers to transition.

Navigation is a short term (120 days or less), solution focused flexible service intended to help members of the community who are experiencing temporary instability find services and resources in the context of a plan.

Independent Living Center - provides advocacy, peer counseling, information and referral, independent living skills and personal assistance in the home.

The Disability Action Center provides advocacy, information and referral, peer support and independent living skills training to people with disabilities and the community. Personal assistant services in the home are also available.

ADA, fair housing, access issues/discrimination, Independent living issues, transportation, personal assistants, sign language interpreters, community services, coping strategies, Social Security, vocational rehabilitation, Medicare and Medicaid, and assistive technology for people with hearing impairment.

Personal Assistance services in the Home are available.

Treatment and Recovery Clinic (TARC) strives to provide a holistic approach to substance use disorder recovery, Mental health management, family healing and the development of associated competencies through the use of community based Behavioral Health Services.

TARC provides Intensive Outpatient (IOP)Substance Use Disorder Treatment, Recovery Support Services (RSS), Mental Health Services, and Life Skills Education to adolescents (ages 11-17), adults, and families.

TARC: Assists clients and their families in developing the necessary insight into the nature of addiction and mental health. Through the use of groups, individual sessions, and family therapy, clients and their families build skills necessary to live in recovery and resilience.

Recovery Support Services (RSS) includes: Drug and Alcohol Screening. Life Skills Education and Guidance, Transportation and Case Management.
