New Portal Partner States!

We’re very pleased to begin to have a presence in the eastern part of the United States, and to be adding services information for the intermountain west.
Rhode Island
We’re delighted to announce that Rhode Island is an official partner state with many RI community and non-profit service providers in the RI Medical Home Portal services directory.  They are also in the process of adding RI pediatric subspecialty provider data, which should be live in the next week.
Nevada
Nevada isn’t far behind - community service provider data from Nevada 2-1-1 will be available in the NV Medical Home Portal service directory in October.

UCCCN Learning Session on Oct 12th. Register by October 4th!

The Utah Children's Care Coordination Network (UCCCN)’s Fall Learning Session will be held on Wednesday, October 12th, 2016, from 9:00 – 11:30 am at the University of Utah’s Officer's Club, 150 South Fort Douglas Blvd.  Our topic is “Improving Pediatric Outcomes through Care Coordination.”  The event’s primary objective is to provide pediatric medical homes and other pediatric-focused organizations with information and tools to improve outcomes through care coordination.  The event falls during the U of U’s Fall Break, so traffic to the campus will be greatly reduced.  Parking is free, however, carpooling is encouraged.  A light breakfast will be served beginning at 8:30 am.
