• Diagnosis & Conditions landing page alpha columns – a cleaner, quicker way to see all the Diagnosis modules on the Portal

The Utah Pediatric Partnership to Improve Healthcare Quality (UPIQ) has been providing a “speed resourcing” (think speed dating) segment in their learning sessions for several years and it has become the most popular part of these day-long focused sessions. It gives participating pediatric practices an opportunity to talk to a “live person” and provides the opportunity for the teams to get their questions answered on the spot. Practices also like that everyone on the team gets to talk to the individual resource at the same time.

Competent mental and behavioral health services and providers for children are in high demand. The Portal feels a brief but in-depth “face-to-face” look at service providers in this field could help primary care clinicians and Medical Home teams in Utah in referring patients and their families to valuable professional and community services. The Utah Chapter, American Academy of Pediatrics provided funding to create short “speed resourcing” videos asking key questions of behavioral health providers.

We feel they represent a reasonable range of provider types for the Salt Lake and Davis/Weber (and in some cases, beyond) areas. They include:

• Neurobehavior HOME Program
• The Polizzi Clinic
• The Utah House
• Utah Behavior Services
• Valley Behavioral Health
• Wasatch Pediatric Neuropsychology

The Utah Children's Care Coordination Network (UCCCN) was formed late last year to be a source of information, resources, tools, expert advice, and peer learning and support for your staff members who help coordinate the care of your patients. The UCCCN evolved from a Medical Home Demonstration project, led by the Utah Pediatric Partnership to Improve Healthcare Quality (UPIQ) and the Medical Home Portal, and is a partnership with the Utah Utah Bureau of Children with Special Health Care Needs (UDHHS). UCCCN members attend monthly meetings (3rd Wednesdays at 8:30 am) where they've learned about:

• What is Care Coordination?
• Identifying Patients Needs
• Family-Centered Care in the Medical Home Model
• Population Management and Registries
• Behavioral Health

And have learned from the following organizations about their services:

• Utah Medicaid
• Pediatric Neurology & Comprehensive Care Clinics, Eccles Outpatient Building Tour
• Utah Parent Center
• Bureau of Children with Special Health Care Needs
• University Developmental Assessment Center
• Shriners Pediatric Orthopaedic Services and Hospital Tour

Meetings also allow UCCCN members to share challenging cases, great ideas, unique resources, and lessons learned. The Medical Home Portal is used as a resource for information and finding services. The UCCCN offers its members an email listserv that can be used to seek answers to questions, share ideas, and find support in between meetings.
