March 23rd UCCCN Learning Session
The Utah Children's Care Coordination Network Spring Learning Session will be held on Friday, March 23rd, 2018, from 8:30 – 11:30 am at the University of Utah’s Officer’s Club. Our topic is “Care Coordination: What do Families Value?”
Our panels include parents of children with complex, chronic conditions and clinicians in pediatric primary care, sub-specialties
and mental health. We hope that participants will gain a better understanding of the goals and challenges of, and potential
strategies for improving, coordination of care for children and youth with special health care needs. This session should
be valuable for anyone involved in caring for pediatric patients – clinicians, care coordinators/managers, administrators,
insurers, other health care providers.
The event is during the U of U’s Spring Break, so traffic to the campus will be greatly reduced. Parking is free, however,
carpooling is encouraged. A light breakfast will be served. Please register to attend at UCCCN 2018 Learning Session Registration by Monday, March 19th. There is no cost to attend. Please have each member of your group register individually.
Portal Advisory Board
The Medical Home Portal has or will soon have agreements in place with the following partner states: Utah, Montana, Nevada,
New Mexico, and Rhode Island. We recently held our first Portal Advisory Board (PAB) with representatives from each. The
purpose of our PAB is:
- To seek feedback and guidance on how to maintain and continually improve the Portal
- To share analytics reports and assess the effectiveness of the Portal
- To develop, evaluate, and prioritize ideas for new features and improvements
- To learn from each other how to use the Portal to improve care and access to services
- To seek guidance and support related to Portal policies, sustainability, and direction, including both near- and long-term goals.
Initially, we’ll meet (virtually) every other month. We look forward to the energy and new perspectives this group will provide
for the Portal.
