Redesign!
We're very excited to announce the newly redesigned Medical Home Portal site, which you can check out right now. A few things
to look for include the option to hide/unhide the left menu on content pages, the responsive design (which means the site
works great on small screens like tablets and smart phones), and splitting up of Services and Other Resources into their own
distinct sections. Please give us your feedback about anything that doesn’t work or that you'd like improved. (The Feedback link is now at the bottom of every page in the green bar.)

Utah Children’s Care Coordination Network
The Utah Children’s Care Coordination Network (UCCCN) is well underway with 3 monthly meetings under our belt this year.
Members include those in care coordination roles from 12 practices and 18 clinic locations, plus some health departments and
health systems in and around Utah. Meetings focus on key topics recognized as components of quality care coordination in
the Medical Home model, and include the opportunity to brainstorm about cases and to share local resources with each other.
If you have a care coordinator or employees fulfilling those duties who could benefit by attending these free meetings, have
them contact us to join! Meetings are held on the 3rd Wednesday of each month from 8:30 – 10 am. Locations rotate between
the Eccles Outpatient Building on the U of U Health Sciences campus or the IHC Doty Education Center in Murray.
Next up for the Portal
- With the redesign (Phase I) complete, we will turn next to making customized service provider lists available.
- Search capabilities will continue to be improved, and better ways to show results will be developed.
- Nevada will be joining us as a partner state in the next few months – we’re very excited to be working with the Nevada Title V and 2-1-1 organizations.
