We heard your feedback and are making some changes to the Portal's Diagnoses & Conditions modules. These include less redundancy and a one-page layout, plus some new navigation features in the left menu. We'll be moving all our modules to the new format over time, but you can see the changes in the following that have also been recently updated:

• Cornelia de Lange Syndrome
• Fragile X Syndrome
• Headache: Migraine and Chronic
• Neurofibromatosis Type 1
• Seizure Disorders (FAQ)

The National Center for Ease of Use of Community-Based Services features case studies on community-based organizations, providers, Title V programs, and family organizations that are using innovative ways to make services easier to use for CSHCN and their families. They recently featured the Medical Home Portal - see the National Center for Ease of Use of Community-Based Services - Medical Home Portal Case Study ( 294 KB). The purpose of these case studies are to increase awareness of these programs, provide concrete examples of partnerships, activities and lessons learned, and identify programs that can be replicated in other states.

Continuing Medical Education: The Portal has three Continuing Medical Education courses offered free-of-charge:

• Headache Continuing Medical Education
• TBI Continuing Medical Education

For Your Breastfed Patients and their Parents:

The Pregnancy Risk Line is adopting the brand name "MotherToBaby UT." MotherToBaby, an affiliate of the Organization of Teratology Information Specialists (OTIS) is dedicated to providing evidence-based information to mothers, health care professionals, and the general public about medications and other exposures during pregnancy and while breastfeeding. The new MotherToBaby branding aims to help families and professionals more easily find and better understand the service. For Your Foster Care Patients:

The Utah Department of Health’s Fostering Healthy Children Program (UDHHS) (FHCP) works with the Division of Child and Family Services (DCFS) to promote the health and well-being of children in foster care.

Each child in the Utah foster care system, regardless of the child's health, is assigned a registered nurse to oversee his or her health care needs. FHCP nurses, co-located with DCFS caseworkers:

• maintain health history information on each child
• coordinate medical care with various caregivers
• link each child to medical, dental, and mental health services including preventive health care and all necessary follow-up care
• track, monitor, and evaluate the services received

The nurses speak with and provide education and consultation to the biological parents as needed. Biological parent involvement is encouraged when possible and the parents are notified of the doctor visits so they may attend. Nursing staff will identify and work with the child/family’s medical home. Where one is not available, the nurse helps to establish a medical home for the child.
