Service Provider Lists are Live!
We are pleased to announce that you can now create your own custom lists of local service providers using the Portal’s database.
Once created, you can:
- email, print, or save lists as PDFs
- modify existing lists
- send lists to families to help address current needs and care plan updates
- maintain lists for your practice of regularly-referred to providers and organizations
Existing lists will be updated with any new information whenever the database is updated. The Portal is Utah’s Shared Resource for Children & Youth with Special Health Care Needs
Utah is one of 16 states to receive a Health Resources and Services Administration (HRSA) State Implementation Grant for Enhancing
the System of Services for CYSHCN through Systems Integration grant. The Medical Home Portal (Portal) has been chosen as
the ‘shared resource’ for Utah’s Utah Bureau of Children with Special Health Care Needs (UDHHS) Systems Integration project. The Portal strives to be a "one-stop shop” where practices and families that care for children
and youth with special health care needs (CYSHCN) will find information, resources, and how to connect with community and
professional service providers. To meet the Shared Resource aims of the grant*, the Portal has implemented the following
improvements:
- improved design
- improved navigation
- customizable service provider lists
And continues to work on these enhancements:
- improved search
- state-specific content
- exploring options for “live person” contact
* By October 2017, 50% of families and medical home providers of CYSHCN contacting a shared resource (SR) for a needed specialist,
support or service, will obtain a needed specialist, support, or service.
