Your Medical Home

One of the most visited pages on the Medical Home Portal is the Working with Insurance Companies page with over 1,500 visits each month. These letters from the medical home can help patients access needed evaluations, treatments, and equipment that may be otherwise unavailable. Identifying a team member in the medical home that has expertise in writing these types of letters can save the primary care clinician time and frustration. Saving copies for use as future templates or to document a series of requests can be helpful for the medical home and families.
